Unprocessable — incomplete or invalid information

The claim could not be processed at all, so it carries no appeal rights. It must be corrected and resubmitted.

Why you actually get this

  • A required field is missing or malformed.
  • An identifier is invalid.
  • The claim failed front-end edits before reaching adjudication.

How to check whether it’s correct

  1. 01Identify which element is incomplete — other remark codes on the same remittance usually say.
  2. 02Correct and resubmit rather than appeal.

There is nothing to appeal, and that is the important part: an unprocessable claim was never adjudicated, so appeal rights do not attach. Treating it as a denial and filing an appeal wastes the filing window. Correct and resubmit instead.
