CARC 49
Routine or preventive, and not covered as billed
The payer treats this as a routine or screening service outside the covered benefit.
Why you actually get this
- A preventive service billed outside the plan's covered interval.
- A diagnostic service performed alongside a preventive exam and absorbed into it.
- A genuinely problem-focused service coded as routine.
How to check whether it’s correct
- 01Check the plan's periodicity schedule for this service and age band.
- 02Establish whether a problem was addressed that would make this diagnostic rather than routine.
- 03Confirm whether preventive and problem-focused elements were separated in the documentation.
Sometimes worth appealing
Where a problem was genuinely addressed and documented separately from the preventive visit, both are billable and the appeal is strong. Where the service was routine and fell outside the covered interval, it is patient responsibility — and the patient is rarely expecting the bill, so tell them early.
