CARC 109
Wrong payer entirely
This payer is not responsible for the claim — it belongs somewhere else.
Why you actually get this
- The claim was routed to the wrong payer identifier.
- The patient is covered by a managed care plan rather than the plan billed.
- The service falls to a workers' compensation or liability carrier.
How to check whether it’s correct
- 01Confirm the payer identifier against the card presented on the date of service.
- 02Establish whether a managed care plan administers the benefit.
- 03Rule out an injury-related claim that belongs with another carrier.
Rarely worth appealing
There is nothing to appeal — the claim needs to go to the right payer. Speed matters, because the timely-filing clock at the correct payer has been running the whole time.
